Instructions for Program Enrollment

Individuals looking to participate in a NPGA Registered Apprenticeship Program must first apply using the forms below.  Applicants should submit all appropriate and completed forms to NPGA.

• Individual Enrollment Form
• U.S. Department of Labor Apprenticeship Agreement (Form ETA 671)

  • Service Technician
  • Transport Driver
  • Utility/Service Driver
