HAZMATICS Temporarily Offline

The Office of Hazardous Materials Safety (OHMS) of PHMSA notified us that HAZMATICS (the Hazardous Materials Incident Communication System) is temporarily offline due to technical difficulties that prevented PHMSA from maintaining the integrity of the information.  HAZMATICS serves as the online system for Form 5800 incident reporting as required by 49 C.F.R. 171.16 Detailed hazardous materials incident reports.  The search tools of the system are also temporarily disabled.  PHMSA enforcement is aware of the offline status of HAZMATICS, and is taking that into consideration when reviewing Form 5800 submissions.

PHMSA Home Page
The home page instantly informs that HAZMATICS is temporarily offline. When you click on the image, it will direct to the message stating it is offline as well as the numbers for the HAZMAT Info Center (202-366-4488 or 800-467-4922) that will be able to answer questions or provide assistance. Please note that if you click on DOT 5800.1 it will direct you to the fillable PDF DOT 5800.1 form. The PDF form can be submitted by clicking the submit button on the last page, which will send it to a mailbox managed by the Data and Statistics Division of OHMS. Alternatively, the PDF form can be emailed to PhmsaHAZMATICS@dot.gov.

Hazardous Materials Incident Statistics
The 10 Year and Yearly Reports links have been replaced with a message under “Available Reports” stating that it is temporarily offline until further notice.

Clicking on the link above will direct you to the message stating that the hazmat online report search tool is offline until further notice.
